About

We are a case management provider for high risk pregnant women of any age and children with a health condition or health risk ages 0-20 years. We help our clients gain access to social, medical, educational, and other needed services. You must be eligible for medicaid or currently have medicaid for our services.

Office Hours:
Monday thru Friday 8am-5pm.


Our services

Support

We are here to support you every step of the way. Our team offers support with applying for Medicaid enrollment; SNAP and TANF benefits; housing options; substance abuse help; and medical transportation.

Health

We can help you with finding a doctor, a specialist, or dentist who accepts your insurance plan. We can also help you schedule your next appointment.

Education

Our team can assist with access to Early Childhood Intervention (ECI). We can go with you to ARDS meeting and parent/teacher conferences at your child's school.

Advocacy
